Our in-home Disability Care program helps propel adults living with a physical, intellectual, developmental and/or sensory disability toward a happier, healthier, and safer quality of life, in the comfort of their home and community. PARKINSON’S CARE
We recognize that each person and family living with Parkinson’s disease is unique, and that a complex, progressive illness such as Parkinson’s requires an interdisciplinary approach and ongoing communication between care providers, medical professionals and family members. From moderate personal assistance to full-time care, we can support your in-home Parkinson’s care plan to help you meet your particular goals and needs, including help with healthy meal planning, transportation to and from appointments, and medication reminders. DIABETES CARE
