Kim Paul of SYNERGY HomeCare recently presented to the St. Joseph Community Health Resource Forum to share how we work to support the senior community and those who work with them. She shared that the our most valuable contribution is to help reduce trips to the hospital. By assisting our clients with nutrition, fall prevention, staying hydrated, taking medications on time, and transportation to their physician appointments, we can greatly reduce repeat trips to the hospital.
