Tom Yetzer – Owner

Tom Yetzer - SYNERGY HomeCare of DenverI started SYNERGY HomeCare out of my own very real passion for home care.  It is for you, and other families like yours, that I built SYNERGY HomeCare into the most trusted home care company in Denver.  I believe in the meaningful options and choices that home care creates for those seeking long term care and in-home services.  This is truly a family company with help and support from my family and a strong belief all of our employees and clients are part of our family. “Welcome to our family”

Shanna Tourtlotte – Owner

Shanna Tourtlotte - SYNERGY HomeCare of DenverHi, my name is Shanna Tourtlotte and I am the co-owner at SYNERGY HomeCare of Denver. My dedication and desire to help people brought me to the SYNERGY HomeCare family in 2009. Over the years, working within the Denver health care services industry, I have learned something new each day and I am continually grateful for the lessons about life, courage, adaptability and the relationships that have been developed along the way are life-long. Kim Paul – Community Liaison

Kim Paul - SYNERGY HomeCare of DenverI am Kim Paul, Community Liaison with SYNERGY HomeCare of Denver since 2014. It was my personal experience with needing care in my own home that brought me to this career. My late husband Brian was a quadriplegic for 10 years, so I personally know and deeply understand the value of having some extra help at home. So many people are challenged with taking care of a loved one and maybe raising children at the same time. SYNERGY HomeCare allows the “sandwich generation” to juggle work, parenting, and parents, and know that our family members are being well taken care of at the same time. When the opportunity to work with SYNERGY HomeCare came my way, I jumped at the chance. There are lots of options out there for home care for family members, parents, grandparents. But SYNERGY HomeCare stands out as the quality organization I would trust in my own mom’s home. I am excited to be a part of the team and to help make sure that our clients and their families get the care they need.

Alice Kershaw – Community Liaison

Alice Kershaw - SYNERGY HomeCare of DenverHi, my name is Alice Kershaw, Community Liaison with the SYNERGY HomeCare of Denver team.  I am honored to be one of your first points of contact and look forward to the opportunity to share all of the wonderful services that SYNERGY HomeCare of Denver offers. As your Community Liaison, you will see me out in the community performing quality visits, meeting and consulting with families and working to create plans tailored to the specific needs of each family. By taking a consultative approach to understanding each family’s goals and objectives, I aim to speak on your behalf and help you feel more secure and confident in your care choices. 

When I am not out mingling with the community, you can find me spending time with my husband and 2 elementary-aged kids. I love hiking, gardening, and trying new restaurants. I am passionate about animals and typically have 1-2 foster dogs rehabilitating at the Kershaw house at any given time. I believe that my friends and family would say that I am a very compassionate person and I am so excited to be a part of a team with the same values. Brea Yapuncich – Director of Operations

Brea Yapuncich - SYNERGY HomeCare of DenverHello, all. I am a Washingtonian born and raised in Spokane, WA. I graduated from a local state university with a Bachelor of Science in Geology (rocks, rock!). During the time spent on my degree, I was a caregiver for a state-run facility for developmentally disabled adults. After graduation, my husband and I packed up our belongings and moved to the Denver area. Since then we added one to our clan: a young lab named Iggy, short for Icarus. I’m very excited to be here at SYNERGY HomeCare and look forward to continuing work with our caregivers and clients.

Giselle Sotuyo – Director of Human Resources

Giselle Sotuyo - SYNERGY HomeCare of DenverGreetings! I was born and raised in Southern California and have been living in Colorado since 2007. For the past eight years, I have dedicated myself to helping families receive the best care and caregivers for their loved one. When not at work I enjoy spending time with my family, significant other and my dog. I am a huge music and food lover and I like to attend concerts and discover some of the best eateries in Colorado. I am grateful to be a part of the SYNERGY HomeCare team and look forward to meeting and working with our clients and caregivers.

Lisa Crogan – Client Care

Lisa Crogan - SYNERGY HomeCare of DenverHi Everyone! My name is Lisa Crogan and I am the Client Care team member in the SYNERGY HomeCare of Denver office. I assist with care coordination and scheduling, which appeals to my love of order and my penchant for completing daily tasks and solving puzzles. I came to caregiving after looking after my mother through her Alzheimer’s. Taking care of the needs of others comes to me naturally. I am the mother of seven children, an author, and a life coach in the making. My free time is spent with my children and my boyfriend, dancing, hiking, and generally moving at the speed of a stampeding buffalo. I believe that life is what you make of it and that we must learn new things daily. Working for Synergy has been a tremendous blessing to me, and I am thankful to be part of a cohesive, supportive team of wonderful people.

Kiaren – Acosta – Human Resources/Recruiting

Kiaran Acosta - SYNERGY HomeCare of DenverHi, my name is Kiaren and I was born and raised in Denver, Colorado. I attended college and earned a certificate in Pharmacy tech and later went to college to become a Nutritional Therapist but never was fully satisfied with my path. Later, I came to a realization that my heart is with the elderly and quickly made caregiving and home care my full-time career.  Prior to joining the SYNERGY HomeCare office team, I worked in the field as a caregiver!  I started my role in the Operations department but quickly learned that I enjoy making relationships and finding great candidates to care for the elderly, so I moved into the HR department.  I love getting the elderly the love and care they need and deserve and so happy to be working every day in this industry! Jenn DeLeon – Client Care

Jenn DeLeon - SYNERGY HomeCare of DenverHola/Hello, my name is Jennica, but I go by Jenn.  Born in Guatemala, raided in California but life brought me to Colorado in 2017 and I could not be happier. Growing up with a big family, caregiving became part of my life as a young girl, whether I was watching over my younger siblings or babysitting my cousins. I went to Culinary School after high school and realized that although I love to cook, I didn’t want that as a career for myself. Years later I’m working in an office for State Farm Insurance, working day by day, where an opportunity suddenly presented itself and I left my office job to be the nanny to a wonderful little girl. My love for caregiving only blossomed from there and I went on to care for other families until the kids were older and started school. Kids grow up really fast and my love for caregiving only grew more. Once in Colorado I started caregiving for our beloved senior citizens through SYNERGY HomeCare and felt such gratitude. Being part of SYNERGY HomeCare has made it possible for me to continue helping others through caregiving and it has brought joy into my life. I look forward to being that helping aspect and serving you and your loved ones through SYNERGY HomeCare.

Albernice McFate – Client Care

Albernice McFate - SYNERGY HomeCare of DenverMy name is Albernice McFate, born in Germany and raised in Maryland, England, and now reside in Colorado. Healthcare has always been a passion of mine. Working in the pharmacy at an early age gave me the knowledge of how much help I can be with just dispensing medicine correctly. That led me to work in pain clinics, being an oral surgery and maxillofacial assistant, and ultimately working in the operating room. Today I have taken on a new entity of care and I am excited to be part of this team and to help all the families that come across my path.
