Connect America Medical Alert – We partner with Connect America Medical Alert to provide medical alert devices that work with any phone line or cellular service. Some SYNERGY HomeCare clients may qualify for a free phone line unit to place in their home. Other models are available, including both a cellular and a GPS-based unit, and an automatic fall detector unit. A medication dispensing option also is available to ensure clients take their medications when instructed. 

Recover Health for Skilled Health Care – We partner with Recover Health to provide skilled services to our clients, including nursing, and physical, occupational and speech therapies. Working together, SYNERGY and Recover Health can fulfill a full range of home care needs.

Amramp for Wheelchair Home Access – Amramp is a leading provider of modular wheelchair ramps and other accessibility solutions, including stair lifts, grab bars and automatic door openers. Modular ramps are available to rent or buy and can be installed in less than 24 hours. With Amramp, everything is accessible.

Struthers Parkinson's Center – Part of Park Nicollet, Struthers Parkinson’s Center offers a full range of medical management and quality-of-life services for people with Parkinson’s disease and related movement disorders.  It addresses patients’ needs through a comprehensive assessment, interdisciplinary treatment, support, research and education for both patients and care partners. Together, we’re able to provide best-in-class home care to Parkinson’s patients.


Gentle Transitions for Move Management Services – Gentle Transitions is a nationally recognized pioneer in the delicate art of moving seniors, focusing on the often overwhelming issues older adults and their families may face when making a move. Gentle Transitions can handle any move, big or small.

Hospice and Palliative Medicine – St. Croix Hospice provides transition management for you or your loved one in the face of changing patient healthcare journey.
