

Dementia Care at No Cost to You
About 6.7 million Americans currently live with Alzheimer’s disease or another form of dementia. At SYNERGY HomeCare, we understand that dementia takes a toll not just on the people living with the disease but also on their loved ones and caregivers.
To better support those affected by an Alzheimer’s or dementia diagnosis, the Centers for Medicare & Medicaid Services (CMS) has launched a new payment initiative called Guiding an Improved Dementia Experience (GUIDE), also known as the GUIDE Model.
SYNERGY HomeCare has partnered with CMS-approved participant organizations to provide our eligible clients access to comprehensive, coordinated dementia care at no cost.
To qualify for no-cost respite care, individuals must meet the eligibility criteria listed below. Eligible individuals could receive 72 hours of complimentary home care per year.
Who is eligible? Anyone who:
- Has moderate to severe dementia
- Is enrolled in Medicare Parts A & B (traditional Medicare)
- Is not enrolled in a Medicare Advantage (MA) plan
- Lives at home or in a senior living community, including independent, assisted living, or memory care facility
- Is not living in a nursing home as a long-term care resident
- Is not currently receiving hospice care
