What is client centered care?<\/strong><\/p>\n When meeting with a client and their family, it is important for us to use a holistic approach, keeping the \u201cbig picture\u201d in the forefront. We ask the client what their main goals are in the near future, what are they hoping to achieve? Client centered care is a way to help promote meaning and purpose in daily life. Person centered care allows for the client to have autonomy while directing their care and services.<\/p>\n We look at the client\u2019s needs and desires.<\/em> How can we, as a caregiving agency, help them attain these goals? For each individual, this could look very differently. Maybe a client is hoping to regain independence after a fall or surgery or trying to increase their caloric intake. We look at how we can most effectively help them achieve that goal.<\/p>\n Another example is a client who has been home bound and is looking for increased socialization. We would look at how we can best facilitate that socialization in a safe and respectful manner while still helping the client with their other needs, running errands, meal prep and mitigating fall risk for example.<\/p>\n The person centered approach is designed to be responsive to the client\u2019s changing needs. Once a plan is in place, it is naturally flexible and easily adaptable.<\/p>\n We strive to be a member of the client\u2019s overall team, inclusive of doctors, loved ones and caregivers \u2013 all working to the same goals, as set by the client.<\/p>\n Please reach out to us today if you need care for yourself or a loved one.
