Job Summary
Synergy HomeCare, the most trusted name in non-medical home care, is currently seeking per diem RNs to conduct in-home patient assessments in 1) Manhattan, 2) Brooklyn, 3) Queens, 4) Bronx. This is a wonderful opportunity to supplement your income. Home assessments generally take place on weekdays and/or evenings, and occasionally on weekends.
***MUST HAVE FLEXIBLE AVAILABILITY AND BE WILLING TO TAKE SAME DAY AND/ OR NEXT DAY ASSIGNMENTS***
RNs are paid on a per case basis and work as per diem employees. Nurses are generally assigned to 2-8 cases per month. Each in-home visit takes approximately 45 mins to 2 hours depending on the complexity of the case, familiarity level with paperwork, and if the case is a start of care vs re-assessment.
Also, there are assignments available for supervision of HHAs (30 mins) and medication pre-pours (30-60 mins).
Synergy is a NON-MEDICAL private home care agency. We do NOT perform any skilled nursing tasks (no wound care, IV meds, blood draws, etc). Nursing duties include a thorough history and physical exam, medication reconciliation, evaluation of home for safety, and assessment/ supervision/ training of HHA.
Locations: Manhattan (primary), Brooklyn (Secondary), Queens (Tertiary), Bronx (Rarely)
Required experience:
- *** Willing to travel to Manhattan or Brooklyn for same day or next day assignments (Monday – Sunday, 8 am – 6 pm) ***
- Access to a scanner or fax machine, and the ability to transmit assessments to the office within 24 hours of completion
- Acute care, acute rehab, subacute rehab, geriatric care facility, wound care, nursing home, or hospital experience is a plus.
Responsibilities and Duties
Responsibilities include:
- Perform start of service evaluations on new patients
- Perform in-home assessments and subsequent re-assessments of existing patients
- Develop and implement treatment plans in conjunction with the patient’s authorized medical practitioner
- Complete and submit assessment paperwork in a timely and organized manner
- Communication with RN Supervisor via email, fax, and phone
Qualifications and Skills
Requirements:
- NYS licensed and currently registered as a Registered Professional Nurse (RN)
- Graduate of an approved school of registered professional nursing (RN)
- BSN or MSN degree is a plus.
- Ability to interpret documents such as safety rules, operating, maintenance, and procedure manuals
- Strong customer service, problem-solving, organizational, written and oral communication, and interpersonal skills
- Must be able to provide verifiable professional references
- Must have regular access to the internet/email/fax and a mobile phone
- Attention to detail
- Legible handwriting
- ***RN should have a flexible schedule and the ability to take same-day assignments (RN assessments generally take place M-F between 8 am-6 pm. At this time we are NOT looking for RNs with only evening and/ or weekend availability.
