POSITION TITLE: RN Assistant Care Manager
POSITION SUMMARY:
The Assistant Care Manager is responsible for developing, updating and maintain patient care plans, and coordinating with the home office to ensure these plans are accurately followed and sufficient for the patient’s on-going needs. The Assistant Care Manager is expected to spend a significant amount of time visiting the patient’s home and medical centers.

QUALIFICATIONS:
- Current RN license in good standing
- BSN/MSN
- Minimum (2) two years recent and verifiable clinical and managerial experience within a Home Care setting
- Cheerful personality and good communications skills (bilingual in Spanish and English a plus).
- Willing to meet with and speak with patients, their family and their medical professionals to develop, update and maintain proper care plan.
- Strong assessment and organizational skills
- Prior supervisory experience with field staff, and experience with clinical oversight, monitoring quality assurance, and overall case management
- Demonstrated successful track record of working independently
- Ambition to grow and advance beyond current position
- Solid communication and PC skills
PRIMARY RESPONSIBILITIES:
- Ensures the quality of patient care and service by using specialized knowledge and judgment to assure that professional standards are in compliance with applicable State Practice Acts and Agency policies.
- Performs or supervises case management activities for the individual patients, including but not limited to, assessments, patient/family teaching and development and updating of the care plan or paraprofessional task sheet.
- Coordinates with home office staff to ensure the appropriate assignment of personnel to meet the specific need of an individual client.
- Answer incoming phone calls from prospective clients and perform initial care assessments of incoming clients.
- Build and maintain relationships with clients, community agencies, and professional organizations.
- Obtains the necessary physician orders for the home care cases as required by State regulations.
- Ensures that changes in the patient’s medical or mental condition are reported to the attending physician and to Agency employees involved in the patient’s care.
- Coordinates arrangements with other health care and equipment providers to assure continuity of care and adherence to the patient’s care plan.
- Maintains compliance to all agency clinical and administrative policies, procedures and regulatory requirements.
- Is responsible for Agency evaluating and supervising all professional and ancillary homecare staff for conformity with Agency policies and procedures, professional standards of home health care, state health statutes and home care requirements.
- Maintains clinical records in accordance with Agency policy as well as state and federal regulations.
- Knows and interprets community resources available for continuity of patient care.
- Participates in quality improvement activities and evaluations of the Agency.
- Investigates problems, irregularities, policy violations and other reportable events implementing the appropriate corrective action and follow-up.

OTHER RESPONSIBILITIES:
- Promotes Agency to the community, participating in community programs with other professionals when necessary.
- Provides for orientation, in-service education, and addition training for Home Health Aide’s, Registered Nurse’s, and Professional Staff Personnel.
- Participate in, plan and carry out community events (Health fairs, walks, & etc.)
- On-call Duties as needed
- Represent Agency before public as required by Supervisor.
- Other duties as assigned by the Supervisor.
FAIR LABOR STANDARDS ACT STATUS: Exempt from overtime. Employees working in this position must be paid a bi-weekly salary and perform the responsibilities listed above to be exempt from overtime payments.
