SYNERGY HomeCare is seeking a part-time or per diem Registered Nurse (RN) to assist the Director Of Nursing in conducting assessments, client, and CHHA documentation. The coverage area will include mostly Morris county and surrounding areas such as Sussex and Warren counties.
Core Responsibilities:
- Conduct initial client assessments and/or every 60 days, develop a plan of care, follow up visits, and quality care calls
- Provide ongoing supervision of CHHA’s, support and evaluation of care
- Client relations troubleshoot issues with clients and/or aides
- CHHA in service and orientation and other functions deemed necessary to fully evaluate and document CHHA performance
Minimum Requirements:
- Minimum of 2 years experience with a BSN or 4 years RN
- Previous clinical experience working within a home care agency setting with solid assessment and documentation skills
- Strong interpersonal skills with the ability to connect with seniors
- Ability to work independently and manage time effectively
- Ability to travel to assigned assessments mainly in Morris county but also can be in Sussex and Warren counties
