SYNERGY HomeCare is a comprehensively licensed (Non-Medicare) in-home care agency whose mission is to enable our clients to live happy, healthy, and independent lives in the comfort and familiarity of their homes. SYNERGY provides primarily Private Duty services for Veterans and people of all ages, but mainly seniors.

Job Title:   Nurse – 0.5-0.8FTE (Requires active Minnesota license)

Office Location:  Roseville, MN
Our clients are located in the central metro area (core cities and near-in suburbs)

Job Description Summary:
The Registered Nurse creates and oversees care plans for home care services. He/She has an understanding of or experience with home health as they will be helping direct services to meet the needs of the individual and their family within the home setting. The RN respects and promotes the mission, vision, and values of SYNERGY HomeCare.

Essential Job Functions/Responsibilities:

1. Completes an initial assessment of the patient to determine home care needs. Documents needs and requests orders. Admits to the appropriate level of service/care
2. Facilitate the completion and updating of the Care Plan to reflect the client’s most current needs.

1. 1. • Makes necessary revisions as client’s needs change
      • Documents/Updates client records

3. Communicates Plan of Care to caregivers (or assigns to another nurse)
4. Provides supervision and training of caregivers per policies and regulatory requirements
5. Responsible for reassessing the patient following a change in condition or discharge from an inpatient setting
6. Knowledge of State and Federal regulations for the delivery of home care services
7. Other duties as assigned

Tasks/Expectations

• In this position, the nurse will report to Synergy’s owner.
• Work schedule is fairly flexible – beyond certain defined core days/hours:
  • Work schedule can be dependent on your availability.  It is expected that the Office will be aware of your schedule and that you can be reached for urgent matters during business hours
  • Scheduled work (training, client visits, etc.) will largely be during Monday-Friday typical business hours. There will be a need for occasional evening or weekend work – such as scheduled training for a caregiver or client intake/SNV.
• Nurses are expected to be in the field routinely, meeting with clients and caregivers. Documentation may be accomplished from the office or nurse’s home.
