Job Description:
- PRN around your current availability.
- Reports to Owner/Administrator
Duties and Responsibilities:
- Make supervisory home visit to each assigned client’s residence at least every 92 days or if Synergy HomeCare of Conyers receive a complaint, ensuring that the client’s needs are met if the caregiver is providing personal care task services for private-pay clients, VA clients, and Medicaid clients
- Plan, Develop and Implement Plan of Care
- Evaluate Plan of Care effectiveness, modify as needed
- Quality Assurance
- Coordinate and Collaborate with various Care Coordinators, Physicians, Communities, Peers, Client families, Office Staff, and Medical Entities
Pay:
- Flat Fee Per Visit + Mileage Reimbursement
Qualifications & Requirements
- Microsoft office skill
- Ability to balance multiple tasks simultaneously
- Ability to effectively handle stressful situations including heavy workload and/or managing client’s expectations.
- Excellent verbal, written, and presentation skills.
Essential Requirements:
- Licensed Practical Nurse (LPN) – qualified by training, education and experience to supervise tasks in accordance with accepted standards of care
- Ability to communicate in a professional manner with clients and clients’ family members
- Document and assess client’s situation and changes in condition
- Honesty, composure, empathy and compassion
- Annual negative TB Test
- CPR certified, First Aid certified
- Ability to work independently
