The CMS GUIDE Model and No-Cost In-Home Dementia Care

The Centers for Medicare and Medicaid Services (CMS) Guiding an Improved Dementia Experience (GUIDE) is a federal program that provides eligible Medicare beneficiaries with up to 72 hours of complimentary in-home dementia care each year at no out-of-pocket cost. Through SYNERGY HomeCare’s GUIDE partners, eligible individuals with a dementia diagnosis receive care coordination and family resources alongside hands-on caregiver support.

What the GUIDE Model Covers

The GUIDE Model provides access to a comprehensive and coordinated set of support that goes beyond a single caregiver visit. The program funds hands-on in-home care from caregivers who specialize in Alzheimer's and dementia, professional care coordination, and family-facing resources written for the realities of memory loss. If approved, GUIDE beneficiaries and their unpaid family caregivers will receive:

  • A care navigator who coordinates dementia care across providers
  • 24/7 access to a dedicated dementia care team

Who Qualifies for No-Cost Dementia Care

The GUIDE dementia care program has specific eligibility requirements set by CMS, and not every family with a loved one living with dementia will qualify. To qualify for GUIDE benefits, your loved one must meet all of the following:

  • Have a diagnosis of moderate to severe dementia
  • Be enrolled in Medicare Parts A and B (traditional Medicare)
  • Not be enrolled in a Medicare Advantage plan
  • Is living at home and is NOT residing in a nursing home, memory care facility, or senior living community
  • Not currently receiving hospice care

Is the GUIDE Model only for advanced dementia?

The program is designed for individuals with moderate to severe dementia. Recently diagnosed individuals with mild symptoms may not qualify for full benefits, though eligibility can change as the condition progresses.

Does the GUIDE Model work with Medicare Advantage?

No. The GUIDE Model is currently limited to people enrolled in traditional Medicare (Parts A and B). Medicare Advantage enrollees are not eligible at this time.
