Licensed Practical Nurse

SYNERGY HomeCare is a comprehensively licensed (Non-Medicare) in-home care agency whose mission is to enable our clients to live happy, healthy, and independent lives in the comfort and familiarity of their homes. SYNERGY provides mainly Private Duty services for Veterans and people of all ages, but mainly seniors.

Job Title:  LPN
FTE: 0.6 (approx; depends on your availability and schedule)

Job Description Summary:
The LPN works under the direction of our Registered Nurse; the RN oversees the client care plans and employee training. 
The LPN has a background in, or experience with, home health as they will be helping guide services to meet the needs of the client and their family within the home setting. The LPN respects and promotes the mission, vision, and values of SYNERGY HomeCare, and assists the RN in helping keep our agency compliant with applicable laws and statutes.

Essential Job Functions/Responsibilities (under the direction of, or coordination with, the RN):

  1. Provides supervision and training of caregivers per policies and regulatory requirements
  2. Completes regular assessments of clients to determine home care needs and plan of care. Documents needs and requests orders.
  3. Reviews client progress notes
  4. Knowledge of State and Federal regulations for the delivery of home care services

Additional Duties:

  1. Actively participates in Quality Management program
  2. Attends and participates in the agency team care conferences
  3. Participates in training of new staff as well as on-going training, as requested
