LPN or RN Field Quality Assurance

Responsible for quality care and reviewing caregiver and client activities, charts, files, and documents to ensure proper high-quality care is being provided. Continuous communication with clients, caregivers, and office staff. Conduct field visits with clients to assess Plans of Care and caregiver performance.  Audit files to ensure completeness as per guidelines, and assure appropriate care is being provided. Facilitate caregiver training programs and assure that caregiver staff is trained and ready and comfortable with service all clients, both Basic and Comprehensive.

Client Management

Quality Program and Client Assessments

    1. Assist Director of Nursing (DON) in managing the Quality Process.
    2. Coordinate with DON regarding elements of the quality control program including but not exhaustive, 5-day, 14-day, 30-day, 90-day quality calls/visits to clients.
    3. Conduct Client Assessments as required.
    4. Communicate Plan of Care changes to appropriate office personnel, DON, and caregivers.
    5. Actively manage customer issues and complaints to resolution.
    6. Effectively communicate with clients and others as directed by management.

Care Coordination

Manage the quality assurance process to assure delivery of the highest levels of customer care and satisfaction

        1. Including caregiver field supervisory assessments, client care assessments, and documentation.
        2. Document Plan of Care Changes in the field and in eRSP and with appropriate management.
        3. Respond to questions, concerns, and inquiries of clients and caregivers
        4. Maintain regular communication with caregivers and clients

Caregiver Training / Development

May include opportunities for facilitating the caregiver training program, including:

    1. Working with DON and coordinate Skills Training dates as appropriate
    2. Work with Recruiting specialist to organize/plan an event, and coordinating participants.
    3. Assuring accurate and updated training materials, (e.g., handouts for documentation standards, RN delegated tasks as appropriate).
    4. Assist with Caregiver mentoring program - Coordinate with recruiting specialist and office manager any assistance to help new caregivers with their transition to SYNERGY HomeCare.
